What exactly is a “concierge” case management service?

Our concierge model is an all-inclusive, high-touch approach. Unlike traditional services where you might interact with multiple people or departments, you work directly with our core team for everything. We handle all logistics, coordination, and support, acting as a single point of contact for your family. This includes 24/7 availability for crises, daily check-ins with the client, and managing all clinical and financial aspects. It’s the most personal and comprehensive level of care available.

Who is this service for?

Our services are designed for families and individuals facing complex challenges related to mental health and/or substance abuse who require a higher level of support than traditional outpatient care can provide. We are an ideal fit for families who feel overwhelmed by managing a loved one’s care and for individuals who need intensive, real-world support to build a stable, independent life.

What happens during the initial assessment?

The initial assessment is a comprehensive, confidential meeting where we listen to understand the full scope of the situation. We meet with the family and, when appropriate, the individual to gather history, identify core challenges, and understand the family dynamics. From this, we determine the appropriate level of service and co-create a detailed, personalized plan of action that outlines the goals, strategies, and expectations for our work together.

Do you work with clients outside of Boca Raton, Florida?

While we are based in Boca Raton, our model is designed to be flexible. We will travel to meet with clients wherever they are, as determined by the needs of the case. Our services include the ability to accompany clients for travel to treatment facilities or to provide on-site support in their home environment, regardless of location.
